This major Not for Profit look to deliver high quality social and support services to a range of people across the community. Having recently taken on some additional packages they are looking to add a newly created Home Care Coordinator role.



Reporting to the Operations Manager you will be charged with delivering in-home care packages to elderly community members. Managing a team of direct care staff you will complete in home assessments, develop care plans and coordinate rosters. Partnering with local health and specialist social care providers, you will ensure clients receive holistic care programme. As an RN you will also work directly with clients on higher level medical issues.



You will need to be a Registered Nurse with nursing management and aged care experience. Highly organised and able to work independently you will understand the importance of high level service delivery and the nuances of in home care.
